Nonfermenters & carbapenemases: 25 questions

The hard half of the guidance — carbapenemase-directed therapy for CRE, DTR Pseudomonas aeruginosa, carbapenem-resistant Acinetobacter, and Stenotrophomonas maltophilia. Pick an answer and the rationale opens underneath, with the guidance question it comes from.

Note
Several answers changed in v5.0 — most sharply for S. maltophilia and CRAB. Worth flagging to anyone working off the 2024 version.

Pairs with Part 1. That set covers general management, ESBL-E, AmpC-E, and CRE urinary infections. This one picks up where it stopped: CRE invasive infection by carbapenemase type, then the three nonfermenters. Every treatment suggestion here assumes in vitro activity of the chosen agent has been demonstrated.
